In accordance with Personal Data Protection Act 2012, MHI Training Master collects, used and discloses your Personal Data such as name and identification numbers (e.g. NRIC, FIN, Passport) for the following purposes; to verify your identity; for processing of SkillsFuture Singapore (SSG) funding applications; for the issuance of individual course completion certificates whereby such data (names and identification numbers) will be printed on the certificate; submission to SkillsFuture Singapore (SSG) for Training Quality and Outcome Measurement (TRAQOM) Survey Initiative; submission of assessment results of Workplace Safety and Health related courses to Ministry of Manpower; audits conducted by Singapore Resuscitation and First Aid Council (SRFAC) and SkillsFuture Singapore (SSG); to verify and process your personal particulars and payments in relation to provision of services to you; compliance with any applicable rules, laws and regulations, codes of practice or guidelines or to assist in law enforcement and investigations by relevant authorities; resolving complaints and handling requests and enquiries; conducting feedback and analysis for the improvement of services provided to you.
MHI Training Master will take reasonable steps to protect your Personal Data against unauthorised disclosure. Subject to the provisions of any applicable law, this Personal Data may be disclosed, for the purposes listed above (where applicable), to the following third parties: SkillsFuture Singapore (SSG); Ministry of Manpower (MOM); Auditors from SkillsFuture Singapore (SSG) and Singapore Resuscitation and First Aid Council (SRFAC); relevant government regulators or authority or law enforcement agency to comply with any laws or rules and regulations imposed by any governmental authority.
By completing and submitting this enrollment form, you are hereby give consent to MHI Training Master to use your Personal Data as set out above. Please note that if you withdraw your consent to any or all use of your Personal Data, depending on the nature of your request, MHI Training Master may not be in a position to continue to provide its services to you or administer any contractual relationship in place.
